1. Acceptance of Terms

By accessing or using Fantastic Forms, you agree to be bound by these Terms of Service and all applicable laws and regulations. If you do not agree with any of these terms, you are prohibited from using this service.

2. Description of Service

Fantastic Forms provides an online form builder platform that allows users to create, customize, share, and analyze forms and surveys. Our service includes both free and paid tiers with different feature sets.

3. User Accounts

To use certain features, you must register for an account. You agree to:

  • Provide accurate, current, and complete information
  • Maintain and update your information to keep it accurate
  • Maintain the security of your password and account
  • Accept responsibility for all activities under your account
  • Notify us immediately of any unauthorized use

4. Acceptable Use

You agree not to use our service to:

  • Violate any laws or regulations
  • Infringe on intellectual property rights
  • Transmit malware or other harmful code
  • Collect data without proper consent
  • Engage in spam or unsolicited communications
  • Impersonate any person or entity

5. Restricted Data Types

Fantastic Forms is a general-purpose form builder and is not certified, audited, or contractually configured to handle the following categories of regulated data. You agree not to use the service to collect, store, transmit, or process:

  • Protected Health Information (PHI) as defined by HIPAA, HITECH, or comparable health-data laws. We do not offer Business Associate Agreements (BAAs) and the service is not HIPAA-compliant.
  • Cardholder data as defined by PCI-DSS (full credit card numbers, CVV, magnetic stripe data). Use a dedicated PCI-compliant processor such as Stripe Checkout.
  • U.S. Social Security Numbers, full driver's license numbers, passport numbers, or other government-issued identifiers, except where strictly necessary and lawful for your business purpose.
  • Children's personal information from users under 13 (subject to COPPA), or under 16 in the EU (GDPR), without verifiable parental consent that you obtain and document outside the service.
  • Classified or export-controlled information subject to ITAR, EAR, or comparable national security regulations.

If you collect any restricted data type in violation of this section, you do so entirely at your own risk and agree to indemnify Fantastic Forms against any resulting claims, fines, or damages. We may suspend any form or account that we reasonably believe is collecting restricted data.

6. Payment Terms

Pro plan subscriptions are billed monthly at $99/month. Payments are processed securely through Stripe. You may cancel your subscription at any time. Refunds are provided in accordance with our refund policy.

7. Termination

We reserve the right to suspend or terminate your account at any time for violations of these terms. Upon termination, your right to use the service will cease immediately.
